but feel free to print and complete your form

Order Form

• Photos will be taken during your normal class time. 
• Ninja athletes should wear their Ninja shirt. All other athletes can wear their favorite leo or whatever they normally wear to class. 
• If you'd like an individual photo of your athlete, the order form + payment must be turned in prior to their class start time. 
• If your athlete is in multiple classes or if you have multiple athletes, each class/athlete must have their own order form.
• Order forms will be available at the gym but feel free to print and complete your form ​in advance.
• Purchase of photos is completely optional!
